

Mission, Vision, and Values
Southern Trinity Health Services has 3 locations locations to serve our patients. Our original facility is along Highway 36 in Mad River one mile away from the county line, our Scotia facility off of Highway 101 just past the Scotia shopping complex, and our newest facility in Weaverville just off Highway 3.
Southern Trinity Health Services (STHS) is a private, non-profit 501(c)(3) Community Health Center. We were also awarded a special designation May 1st, 2005 as a Federally Qualified Health Center (FQHC). The area we serve is federally recognized as a Medically Underserved Area, and has Professional Shortage Area designations for Primary Care, Mental Health, and Dental. The clinic was founded by a group of concerned citizens 26 years ago with a mission “to improve the quality of life in southern Trinity and southeastern Humboldt Counties by providing access to quality, comprehensive, innovative, and integrated primary health care and emergency medical services regardless of ability to pay.”

To create a seamless system of access to compassionate, high quality primary and preventive health care for all residents. We strive to improve the health status within our community, particularly for those residents who are economically or otherwise disadvantaged.

Southern Trinity Health Services Inc. is a FTCA Deemed Facility. This health center is a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n).
This health center receives HHS funding and has Federal Public Health Service (PHS) deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.
