Here's a breakdown of what you need to know

New Patient Registration Information

We are required by state and federal regulations to obtain certain information for our records and proof that we have verification that we have provided you with basic information regarding your care at Southern Trinity Health Services, Inc. This includes our Privacy Practices that you will receive at the time of your appointment.

What to Bring Things You Should Know
  • Contact Information
  • Proof of mailing address
  • Private Insurance, Partnership(PHC) or Medicare Card
  • Records from hospitals or other doctors
  • All current prescriptions
  • Reports on any special procedures you may have had completed such as labs or x-rays.
  • A list of any health concerns you would like to discuss upon patient establishment
It is the patient's responsibility to call the insurance company to verify covereage and provider network participation.

While we try to maintain "In-Network" status with most large insurance providers, we cannot guarentee all insurances will accept us at your "In-Network" rates.

We offer a sliding fee scale that is available to our uninsured and underinsured patients for financial assitance. This sliding Fee Scale is available below to review. To apply, please bring proof of income to your first appointment for verification purposes. 
We understand that a contact number or mailing address is not viable information for all of our patients, however not offering contact information for billed services does not waive patient responsibility for balance due. Statements are mailed monthly for unpaid account balances.

Any unpaid balances will be submitted to collections.
